All new customers must fill out a Credit Application. Please click here to download our Credit Application form. You may fax or email the completed Credit Application to firstname.lastname@example.org.
You can place an order with us in two ways. If you are a repeat customer or if you already know the Item Numbers for your order, you may send us a company Purchase Order to Sales@american-illumination.com.
If you are a new customer or unsure of the correct Item Numbers for your purchase, please click here for an Order Form. Once it is filled out, submit your Order Form as an attachment to Sales@american-illumination.com and one of our Representatives will contact you with a PO Number and order confirmation.
Specialty Project Development
Customers who wish to use our customization services for an item that is not currently offered in our catalogue must fill out a Specialty Project Development Form. It is mandatory that all information is filled out and that you provide a prospected product drawing (by hand or digitally produced) so that we can better serve you.
Return Goods Authorization / Warranty Claim
All items sold by American Illumination, Inc. are under a 5-year limited warranty under normal and recommended use.

If you would like to return an item or exchange an item please download our RGA Form here If you want to make a warranty claim, please download our Claims Form here and submit it with a completed RGA form. You may return the form via fax at (310) 202-6555 or as an attachment to email@example.com. We will review your return request and issue an RGA number, upon which you have 30 days to send us your returned goods. Credit or replacement items will be issued upon inspection of your returns. All returns are subject to a restocking and/or handling fee.
